Submitting app to market place for private use

I am trying to submit my app for marketplace approval. Can someone please guide me on the steps?
Also, even after adding the icons in the information section, it’s not allowing me to submit it saying icon is necessary. Please find the screenshot as attached here.

My apologies Gianni,
The app i am trying to submit is registered from email id: camohealthdev@gmail.com
That’s the mail id for my project. Should I raise request for support from the other zoom profile of mine then?
